REGISTRATION INSTRUCTIONS

  • Complete the registration form.
  • Make check payable to: City of Glendale. DO NOT MAIL CASH. Registration will not be processed without the correct fees.
  • There will be a $25 fee for each returned check. Re-payments will only be accepted by money order, cashier's check or cash.
  • VISA OR MASTERCARD. Complete the credit card information on the next page.
  • When paying in person with cash, correct change is required.
  • Registration is limited. We encourage early enrollment.
  • Registration for Life Long Learning Classes WILL NOT be accepted after the second week of the session. Individual workshop registration deadlines may vary.
  • Activities not reaching minimum numbers by the close of registration may be cancelled and fees refunded.
  • If activity is filled, you will be notified if another activity is added.
  • You may be required to complete a Release of Liability & Indemnity Agreement Form for the program you have registered for.
  • Please note: Registration confirmations are NOT mailed to participants.
LATE FEE REGISTRATION POLICY
  • DAY CAMP - A $15.00 "late registration fee" per child will be charged for payments not received by 5:00 p.m. the Wednesday prior to each week for which you are registered.
REFUND POLICY
  • Full refunds, less a $15.00 administrative fee, will be given if the refund is requested prior to the start of class/program.
  • Refunds after the start of the class/program are contingent upon City Approval and will only be considered through the conclusion of the 4th week of scheduled class/programs. The refund will be assessed a $15.00 administrative fee and prorated based on the number of classes held.
THE GLENDALE COMMUNITY SERVICES & PARKS SATISFACTION GUARANTEE
Satisfaction is guaranteed on all programs. If you are not satisfied with the quality of our programs, for any reason, we encourage you to contact us immediately so that we can work to resolve your concerns and improve the quality of our programs for all participants. If we are unable to resolve your concerns, we will be happy to transfer you to another class, issue you a credit or a refund.

SPECIAL NEEDS ACCOMMODATIONS
The Community Services & Parks Department will provide any reasonable accommodation toward the inclusion of a participant. Please contact our Therapeutic Recreation Coordinator at (818) 548-3785 two weeks prior to the start of the program. Ample time is required to determine the needs of each request.
